  • EMPLOYEE NOTIFICATION OF RECEIPT OF EXPOSURE CONTROL PLAN AND AVAILABILITY OF HEPATITIS B VACCINATION

  • I have received a copy of the McMinn County Board of Education Exposure Control Plan.

    I understand that the Hepatitis B vaccine is available:

    • At no cost to me through the state sponsored insurance plan, and available at area Caremark pharmacies.
    • At no cost to me. If I choose to have the vaccine I will bill McMinn County Schools the amount not covered under my insurance from a source other than McMinn County Schools. (Bill must be submitted with copy of insurance explanation of benefit.)
    • At no cost to me. As an un-insured individual, if I choose to have the vaccine I will contact the Director of School Health (423-745-1612 ext. 1325 or sroberts@mcminnschools.com) to make arrangements.
